- Facilitates resource management activities for inpatients based on identified patients.
- Coordinates patient’s stay from admission to discharge through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction.
Minimum Knowledge, Skills and Experience required:
Education: Require BS degree in Social Work from an accredited college or university and current Alabama licensure.
Experience: Two years hospital experience preferred. Comparable combination of education and experience will be considered. Previous utilization management, discharge planning or hospital based social service experience very beneficial.
Additional Skills/Abilities: Appropriate communication skills, verbal and written. Excellent organizational skills. Strong medical knowledge i.e., medical social work.
Weekend and/or Holiday Rotation may be required.
