Assist patient transition-hospital to home, teaching medication management and self-management skills.
Bachelor’s Degree in social work or health care field with 3 years experience;
Highly motivated, self-directed, good interpersonal, communication & computer skills; own transportation, valid driver’s license/good driving record.
Positions are full time – 40 hours per week
